UnityPoint Health Quality Improvement & Education Coordinator-Hospice in Peoria, Illinois

Responsible for the Hospice quality assurance program, ensuring compliance with acceptable standards of practice, applicable State and Federal laws and regulations. Develops, coordinates, and provides a comprehensive educational program including staff orientation and ongoing education and training of staff.

Delivers to all stakeholder groups state of the art knowledge and expertise in the use and possible adoption of clinical care processes aimed at achievement of optimal outcomes.

  • Demonstrates robust review of current literature and initiation of practices or workflows to consider for implementation based on continuing advancement of Hospice Service.

  • Ensures that all clinical related and other assigned policies and procedures are kept up-to-date and reflective of current practice/regulatory requirements.

  • Deploys policy updates to staff as indicated.

  • Presents regular in-services, programs, and/or courses to the clinical staff.

  • Develops education and applies for CEUs if content is worthy of CEUs.

  • Maintains and deploys up-to-date resource materials for use by nursing staff and patients/caregivers.

  • Writes new policy and procedures reflecting current clinical trends and practices when needed.

  • Provides information and resources in response to staff requests or needs from various input sources.

  • Assists in review and testing of clinical computerized software for appropriateness of use.

  • Maintains thorough knowledge of all products, equipment, and procedures applicable to the patient population that is served.

  • Demonstrates expertise and a thorough knowledge of standards of practice and care through clinical judgment and decision making, and use of creative and realistic solutions to ensure cost effectiveness and quality of care.

  • Demonstrates sound knowledge of Joint Commission and CMS Home Health Conditions of Participation (COP’s) and ensures that current practices are reflective of regulatory requirements.

  • Ensures EMR content is reflective of evidence based practice care plans, protocols, and processes.

Develops, deploys, and monitors effectiveness of staff orientation and ongoing educational programing for all patient care personnel (RN, LPN, CAN, Therapy personnel, and Social Work staff).

  • Demonstrates competency in both the didactic and clinical delivery of education as evidenced by expectations noted in #1 above and by evaluations of selected educational offerings.

  • Responsible for tracking and ensuring completion of annual educational requirements such as Net Learning, CPR, and others as assigned.

  • Collaborates with Net Learning personnel to:

  • Track staff meeting attendance

  • Review, prepare, and assign presentations

  • Evaluates competency of orientees at various stages of orientation period as assigned.

  • Accountable for designing and deploying all mandatory education such as CMS nursing assistant education and other education as assigned.

  • Develops annual education plan and programs to meet needs identified. Works with leadership to prioritize and monitor educational programs. The following input sources can assist in development of program needs:

  • Regulatory requirements/updates/regulatory reviews

  • Performance data from observational awareness, Coding/HIS results, and other inputs as


  • Staff self-assessments

  • Computer software updates

  • Continuously monitors and evaluates various programs and implements improvements based on

    Performs Hospice Quality Assurance Duties

  • Responsible for the implementation and monitoring of the organization’s quality assurance program.

  • Performs and/or monitors results of various outcome and compliance report as assigned.

  • Proposes actions/interventions to improve outcomes based on knowledge of root cause of desired or emerging metrics.

  • Demonstrates use of active networking to gain insight on how others are managing various outcomes.

  • Works collaboratively with team members to problem solve.

  • Demonstrates initiative in the identification of high priority improvement trends.

  • Demonstrates ability of properly prioritizing tasks.

  • Demonstrates active accountability and ownership in quality outcomes and educational training programs.

  • Responsible for the implementation and monitoring of the organization’s quality assurance program.

  • Ensures that there are processes in place to monitor and measure all activities related to QAPI.

  • Provides direction and coordination of quality improvement activities utilizing continuous quality improvement principles and methodologies.

  • Coordinates activities related to clinical record review for all data collection.

  • Works collaboratively with nursing supervisors to assure documentation is complete and consistent with care and reflects legal requirements.

  • Collecting, auditing, tracking, and analyzing healthcare data.

Coordinating and performing staff education on topics related to quality assurance/performance improvement and effectiveness.


Bachelor’s Degree in the field of Nursing Required. Master’s degree preferred


>1 year Hospice nursing


Valid driver’s license when driving any vehicle for work-related reasons. CPR Registered Nurse in the State of IL Valid Driver’s License in State of IL Other: Must have access to own transportation
