This job was posted by : For more information, please see: Chestnut Health Systems is seeking a full-time Community Support Specialist for our Supportive Services for Veteran Families program our Belleville, IL location, servicing the Metro East and St. Louis, MO areas. Prefer military veterans of service or spouses/other immediate family members of military veterans.Job Duties:Complete required data entry, updating SSVF data bases and spread sheets and paperwork in accordance with program and Veteran Administration policies and procedures, including but limited to opening requirements, Housing Stability Assessments, Housing Stability Plans, progress notes and any other funder requirements:o100% of Housing Stability Assessments and Housing Stability Plans to be completed during the first face-to-face meeting with Veteran. o80% of progress notes and any other required data entry or paperwork submitted within forty-eight hours of the delivery of the service.Provide services identified in the Housing Stability Plan. Services to be Veteran driven and meet the program requirements, including providing services to the total number of Veterans identified annually in the SSVF application.Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery; including utilizing recovery language in all applicable documentation.Provide for outreach, engagement, referral, linkage, and advocacy and "warm hand offs" to recommended community resources, including the Veteran Assistance Commission offices and the Veterans Administration, to ensure Veterans and their support systems receive access to recommended services and assistance.Promote positive interactions within Chestnut and with the community, especially with landlords, property managers, Continuum of Cares, and Veteran support and service organizations, to sustain a favorable program reputation.Attend meetings and Veteran staffings, participate in program development activities and provide support and backup to other team members as necessary, including:Attending all SSVF meetings and trainings, as required.Attending all mandatory Chestnut trainings and CEU requirements, if applicable.Provide consultation to Chestnut staff, community providers, families or others regarding referrals, recommendations and progress made by Veterans and their families.Attend Veteran Fairs, job fairs, Stand Ups and Stand Downs, Point in Time Counts and complete street outreach to locate eligible Veterans and conduct eligibility screens as applicable.Maintain SSVF Veteran charts, including opening, closing, filing, and reviewing for accuracy.Provide advocacy for housing sustainability with landlords, property managers, utility companies, Chestnut Credit Counseling staff, employment staff and other community agencies.Maintain and protect confidentiality of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and consumer information.Provide other duties as assigned, required or specifically negotiated with supervisor.
