St. Mary's Good Samaritan Occupational Therapist - Home Health in Mount Vernon, Illinois
Under general supervision, evaluates, plans and implements the treatment of home care patients through the use of various facilitative/inhibitive modalities such as exercise, ADL, orthotics and adaptive/assistive devices designed to increase range of motion, enhance fine and gross motor skills, and improve perceptual and cognitive skills.
Conducts initial evaluations on referred patients and prescribes treatment plans in accordance with department standards.
Schedules patients for treatment in accordance with physician referral, patient assessment and treatment plan.
Conducts treatment sessions individually or in a group.
Performs patient assessments including: biophysical, psychosocial, spiritual, cultural, and age appropriate needs of the patients, including the assessment for signs/symptoms of abuse or neglect in order to develop, evaluate, and modify patient’s plan of care. In conjunction with other disciplines, develops a realistic comprehensive patient/family educational plan, using appropriate resources and methods of instruction and provides patient/family health teaching.
Maintains records of all treatments procedures, including initial evaluations, progress notes, discharge summaries, and daily documentation.
Evaluates patient progress and makes necessary revisions to treatment plan in order to meet the patient’s needs and treatment team goals.
Consults with physicians, family members, and other rehabilitation personnel to assist in coordinating the patient’s total rehabilitation.
Provides education and training to patients and family members in order to maximize patient’s independence at home and in the community.
Assists in the training and supervision of clinical students, and supports treatment plans being carried out by other disciplines as part of the treatment team goals.
Performs related duties to include participating in: special committee work as assigned, quality assessment, and improvement monitoring.
Bachelor’s Degree in Occupational Therapy required
Current Illinois OT license required
Valid IL driver’s license required
Requires effective oral and written communication skills to communicate with customers.
Requires good decision making and problem solving skills.
