Support people to maintain their independence in the community through the development, maintenance, or accommodation of functional daily living skills.

*Essential Functions*


1. Directly deliver services in accordance with applicable Rule 132 program requirements.

2. Schedule and document all billing activities to meet departmental deadlines, time frames, and productivity standards.

3. Maintain proficiency using the most current information and technology systems available to function efficiently within the department.

4. Independently transport consumers in compliance with agency policy and procedure.

*Community Services*

* Independently provide services with consumers in a natural environment.

* Spend 60% of their time providing services off site.

* Respond to crisis calls in the evenings, weekends, or holidays as assigned.

*Education* - Bachelor's Degree in Occupational Therapy.


* Minimum of one year experience working with individuals with mental illness and working knowledge of the recovery model preferred.

* One year case management, linkage, outreach, and/or community support experience preferred.

* Experience with Medicaid Rule 132 documentation and billing requirements preferred.

*License/Certification* - Current license in Occupational Therapy.

*Language* - English as a first or second language.

*Additional Qualifications*

* Valid Illinois driver's license and documentation of current auto insurance, with a good driving record and car available.

* Eligible for and completion of certification process to drive PACE vehicles.

* Must be willing to work evenings/weekend hours as necessary and be on call to respond to emergencies.

* Administrative Code, Part 132 Medicaid Community Mental Health Services Program.

* Proficiency in the use of computers and working knowledge of the following:

*Position Specific*

1. Develop an assessment to evaluate the impact of a person's mental, physical, or medical impairments on maintaining independence in a community setting.

2. Assists the team in the development of appropriate individual goals and objectives that supports gaining or maintaining independence in a community setting.

3. Provide direct services that support individual development of homemaking skills, activities of daily living, independently transport within community, recreational activities, and social activities t rough occupational therapy intervention and approaches within a recovery model.

4. Identifies and procures other necessary supports or accommodations required for main taining independence in a community setting.

5. Manage multiple demands with flexibility in adapting to new challenges while maintaining priorities.

*Customer Service*

1. Respect rights and responsibilities of persons served and function as an advocate.

2. Exhibit professional conduct and appearance throughout all work environments in compliance with department standards.

3. Maintain open and effective communication with agency and provider staff, family, and others in response to consumer needs.


1. Schedule any status billing activities on a daily basis in the Electronic Clinical Record.

2. Complete all billing notes by the end of the business day every Wednesday for the week prior.

3. Adhere to Rule 132 guidelines for completion and standards of clinical documentation.

4. Report and document consumer incidents in the Electronic Clinical Record.


1. Respond to and manage crisis situations.

2. Report persons that present as a danger to self or others to be entered on the FOID Registry by a qualified examiner.

3. Report critical consumer and staff incidents to supervisor immediately.
