          SwedishCovenantHospital PHYSIATRIST in Chicago Illinois United States

          R ESPONSIBILITIES     Essential Functions   Demonstrates a commitment to the
          mission of Swedish Covenant Hospital and demonstrates a service orientation
          and adheres to all responsibilities and standards of the Hospital.    Provides
          on call coverage for holidays and weekends as determined by the Medical
          Director.   Performs inpatient care, outpatient care, and consultations as
          outlined by the Medical Director.   Assumes responsibility for providing PM &
          R services when and wherever needed as determined by the needs of the hospital
          under the direction of the Medical Director.   Provides coverage for
          outpatients and inpatients including Pain Clinic in the absence of another
          Staff Physiatrist or Medical Director.   Develops a good working rapport with
          all staff involved in the care of patients.   Participates in utilization
          review activities of the hospital and as assigned by the Chairman.
          Participates in educational activities as needed for the Nursing Staff, all
          Therapists, and Physician lectures.   Intervenes in the management of
          patients, if required, in emergency situations.   Provides 24 hr PM & R
          coverage for patients when on call and for regular patients.   Serves on other
          committees as appropriate.   Provides input to Medical Director on budgeting
          process concerning purchasing of specialized equipment.   Recommends, develops
          and implements new rehab programs to meet the needs of patients, Swedish
          Covenant Hospital, and the community.    13.   Needed for assisting with acute
          inpatient rehab, outpatient pain/musculoskeletal practice.
          QUALIFICATIONS/BASIC JOB REQUIREMENTS Must be an active member of the Medical
          Staff with medical degrees and licensure as appropriate and Board
          Certification with recognized special training and demonstrated competence in
          Physical Medicine and Rehabilitation.  Ability to maintain good relationship
          with other physicians, clinical staff, and with the nursing and paramedical
          personnel is necessary. EMG efficiency and pain procedures, including facet
          blocks, epidurals and SI injections under fluoroscopy required.  Pain board
          certification a plus.     LICENSE/CERTIFICATION Current Illinois License.
          Board Certification in Physical Medicine and Rehabilitation Pain board
          certification a plus.   RELATIONSHIPS Reports to the Chairman, PM &R;, for all
          medical matters. And to the administrative designee of the hospital for
          administrative matters such as budget, program development and equipment
          recommendations.    ACCESS TO PROTECTED HEALTH INFORMATION Computer system
          access is assigned based on managers access level request and on the minimum
          necessary as described in this job description.  Approval for level of access
          is given by the system administrator or manager of the department that has

          Access level: name, demographic, financial, and clinical   INTERNAL AND
          EXTERNAL CONTACT             Frequent contact with physicians, patients,
          visitors, and hospital staff.   ENVIRONMENT * Possible exposure to chemicals,
          blood/body fluids, air-borne diseases. * Requires full range of body motion
          including handling and lifting patients, manual and finger dexterity and eye-
          hand coordination.  Requires standing and walking for prolonged periods of
          time.  Occasionally lifts and carries item weighing in excess of fifty pounds.
          Requires normal visual acuity and hearing.  Each employee is trained and is
          expected to observe all fire and safety procedures and blood born pathogen and
          hazard communications standards.  Requires working under stress in emergency
          situations and during irregular hours.   The above statements are intended to
