Provides substance abuse counseling services.
Provides assigned patient counseling services and case management.
Systematically and continually assesses the patient's coping mechanisms and progress.
Utilizes appropriate teaching methods and/or referral sources.
Coordinates patient's discharge planning needs and readiness for discharge with other healthcare members.
Willingness to travel in neighborhoods on the north, west and south side of Chicago to engage in home visits with clients as part of mandatory position requirements. This position is based in Chicago.
Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) credentialed from the Illinois Department of Financial and Professional Regulation.
Driver's license required
Bachelor's Level Degree
Bachelor's degree required
- Knowledge and experience with case management, substance abuse, mental health, HIV/AIDS, homeless issues preferred.
